Ive left this in pouch 100% of the time, tossed all manner of stuff in the Stohlquist Decent's pocket and the add-a-buoy has remained in place.
I dont really know if it offers a noticable difference in flotation, of if it will save you in a heavy recirculation, but more flotation is better than less flotation, and it seems like it will get you rolled face up in case you get knocked out while swimming
